All patients are required to go through the cycle of service. The cycle begins on your first visit with a new patient exam and necessary x-rays. It is at this time that patients may schedule a hygiene appointment. It is required that all diagnosed treatment be discussed at a consultation appointment prior to scheduling any dental work. The purpose of the consult is to inform you of your options in your treatment as well as make financial arrangements.

It is important that you arrive on time for your appointment. Your appointment may need to be rescheduled if you arrive more than 15 minutes past your appointment time.

For legal reasons, all patients under the age of 18, must be accompanied by an adult to all appointments, unless a consent form has been signed.

As a courtesy to patients, appointments are confirmed one day in advance. However, you are ultimately responsible for remembering your appointment time and date. If you need to cancel your appointment, a 24 hour notice is required (48 hours for specialist appointments). There is a $100.00 fee for any appointments not cancelled within this time frame.


Patient co-payments are due at the beginning of each visit. For major treatment, previously arranged payments are expected.

There is a 5% professional courtesy for our patients over 60.

Payment is accepted in cash, credit card (Visa, MasterCard, Discover, and American Express). Personal check, and Unicorn Financing. (For more information about Unicorn Financing, see below) There is a $25.00 fee for all returned checks.

It is your responsibility to make sure that you are eligible for insurance coverage.

It is important that you inform the front office staff of any changes in your insurance coverage.

It is your responsibility to inform the staff of any dental treatment rendered in the present calendar year that may affect your insurance coverage.

As a courtesy to you, your insurance company will be billed if necessary.

You will continue to receive a statement until your insurance company has paid their portion of the balance and your account is paid in full.
